Adolph reported that he was born in Tchernigoff (Chernigov?), Russia, but was in 1917 a subject of Germany. He was in fact born in Mogowo, near Nasielsk, in central Poland. In June 1917 he lived at 5927 La Salle in Chicago with wife and children, and worked for the Rock Island Railroad in Blue Island IL.

This happened in the town of Nasielsk on the 1st/13th of April 1894 at 2:00 o'clock in the afternoon. Ludwig Fausak, a farmer from Mogowo, 47 years old, appeared and in the presence of witnesses Johann Ulrich, 64 years old, and Michael Look, 45 years old, both farmers from Mogowo, presented to us a child of the female sex, stating that the child was born in Mogowo on the 7th/19th of March of this year at 1:00 o'clock in the morning from his lawful wife Louise, born Hein, 33 years old. To this child at holy baptism, performed on this day, was given the names Atalie Martha. The godparents were Christian Kurz and Anna Look (Locht?). We declare that, in the presence of witnesses Ludwig Look, 40 years old, and Samuel Schultz, 27 years old, both farmers from Kepa Tarchominska, a religious marital union was contracted between Karl Graf, 34 years old, single, the son of the farmer Daniel Graf and his wife Christine born Neumann - and - Caroline Fausak, 19 years old, single, born in Bledowo and residing in Neu Schwidry, the daughter the farmer from Neu Schwidry Karl Fausak and his wife Louise born Kolm. This marriage was preceded by three readings of the banns proclaimed in the local Evangelical church on the 6th/18th of October and on the next two Sundays. Permission was given orally by the newlyweds parents in person. The newlyweds stated that no prenuptial agreement had been made between them. We declare that on this day in the presence of witnesses Johann Fausak, 30 years old, and Julius Look, 36 years old, both farmers from Mogowo, a religious marital union was contracted between Andreas Bartz, single, 25 years old, born in Debinki and residing in Mogowo, a laborer, the son of the deceased couple Ferdinand Bartz and Caroline Grunke - and - Caroline Fausak, single, 35 years old, born and residing with her parents in Pomocnia, the daughter the Johann Fausak and his wife Anna born Sikinger. This marriage was preceded by three readings of the banns proclaimed in the Evangelical-Augsburg house of prayer in the village of Konary on the 3rd/15th, 10th/22nd, and 17th/29th of October of the current year. The newlyweds stated that they had made no prenuptial agreement. This document after being read aloud, was signed only because those present stated that they don't know how to write. Pastor A.
